Shoulder Orthopaedic Test: Inferior Sulcus Test

The Inferior Sulcus Test is used to assess the glenohumeral joint for inferior instability. Here are the steps to perform the test:

  1. Position the patient standing or sitting with their arm at their side.
  2. Grasp the patient's elbow with one hand and their wrist with the other hand.
  3. Apply a downward traction force on the patient's arm, pulling the humerus away from the glenoid fossa.
  4. Observe for any inferior translation or sulcus formation at the glenohumeral joint.
  5. Repeat the test with the arm in 90 degrees of abduction to assess for multidirectional instability.

The Inferior Sulcus Test is not typically used as a standalone test for diagnosing inferior instability of the glenohumeral joint. Instead, it is usually performed in conjunction with other diagnostic tests and clinical evaluation. That being said, if there is no significant sulcus formation observed during the Inferior Sulcus Test, it may indicate the absence of inferior instability. However, it's important to keep in mind that other factors such as muscle tone, joint laxity, and patient cooperation can affect the results of the test.
