The Grip Strength Bent Arm Wrist Extended Test is a maximal isometric grip assessment performed with the elbow bent and the wrist positioned in extension. The client squeezes the gripper as hard as possible while maintaining the same shoulder, elbow, forearm, wrist and hand position.

This test primarily measures grip force output in a specific bent-arm, wrist-extended setup. It reflects the combined contribution of the finger flexors, thumb position, wrist and forearm stabilisers, elbow position, shoulder position, hand size, grip span, effort quality and confidence.

The wrist-extended position may be closer to the self-selected or mechanically favourable position used by many people during maximal gripping, but it should still be recorded carefully. Different degrees of wrist extension can produce different results. A slightly extended wrist should not be treated as identical to a strongly extended wrist, neutral wrist or wrist-flexed position.

1. Prepare the client

Ask which hand is dominant. Record whether the dominant or non-dominant hand is tested first.

Use 1–2 submaximal practice trials before maximal testing so the client understands the wrist-extended position, handle setting and effort required.

2. Set the client position

Use a repeatable position such as:

  • Client seated upright
  • Shoulder relaxed and close to the body
  • Elbow flexed to approximately 90 degrees
  • Forearm position recorded as neutral, pronated or supinated
  • Wrist extended to a recorded angle
  • Hand holding the gripper without changing wrist extension during the squeeze

5. Stabilise the position

Ask the client to keep the elbow bent, forearm position steady and wrist extended at the chosen angle. The shoulder should stay relaxed and the trunk should not lean.

8. Identify invalid trials

Repeat or mark a trial as invalid if:

  • The wrist moves out of the selected extended position
